    VERIZON CUSTOMERS

    Effective January 1, 2010, all Interstate Special/Switched Access and Local Exchange Reseller customers with Exemption Certifications will be required to recertify. The recertification period will run from January 1, 2010 through March 5, 2010.

    To recertify, an authorized representative of your company must submit a new Exemption Certification at: www.22.verizon.com/wholesale/access/systems/fusfexemption.

    If you do not recertify by March 5, 2010, Verizon will begin billing the FUSF surcharge on your interstate special/switched access and/or local exchange reseller service accounts.

    FLORIDA DEPARTMENT OF REVENUE

    Per the Florida Department of Revenue, effective January 1, 2010 some local communications services tax (CST) rates will change. The local tax rate for CST includes both the local rate imposed and the discretionary sales surtax imposed.

    New York State

    Pursuant to Section 334 of the New York State County Law, Onondaga County has passed a local law increasing the access line surcharge by $0.65, bringing the total surcharge to $1.00. The new surcharge will be in effect as of December 1, 2009.

    Federal Communications Commission (FCC)

    The Federal Communications Commission (FCC) announced that the Universal Service contribution factor for the 1st quarter of 2010 will increase to 14.1% of interstate and, in most cases, international end-user revenues.The FCC's Public Notice reminds carriers that marking up federal universal service line-item amounts above the 14.1% contribution factor is unlawful.

    The District of Columbia general sales and use tax rate has been increased from 5.75 percent to 6 percent, effective October 1, 2009.

    The Public Service Commissin filed an order on December 15, 2009 setting the assessment rate for 2010. The new assessment rate will be 0.002961.
